After you have sent this form off (by clicking the button) you need to send a deposit to the address below in the form of a cheque for £50 made out to Warrington Disability Partnership. Please include your name and address on a seperate piece of paper with your cheque. If you prefer you can call in and drop your cheque off in person, we are always glad to welcome visitors to our Centre for Independant Living.
